- I commission Ever After, LLC to create the portraits shown, and authorize work to commence immediately and agree to pay for these portraits in accordance with studio policies and per any payment plan arrangements I have made.
- I understand that my portraits will be professionally retouched with the utmost care and attention to detail, that all work is 100% guaranteed and Ever After, LLC will do any correction necessary until I am satisfied.
- I understand that additional fees may be charged for anything retouched or redesigned more than two times.
- I understand that portraits ordered for an Album are final and cannot be changed at a later time. (Albums are designed with, on average, 2 images per page.)
- I understand that a Lifetime Warranty is standard with every Wall Art order.
- I understand this recommendation for any prints I might order: Recommendation to protect PRINT(S): If you wish to frame your mounted print(s), do not place the print directly against glass. Either frame it with NO GLASS, or MAT it before framing with glass. This is to prevent any humidity from causing the print to adhere to the glass..
- Social Media Files: I understand these digital files are licensed for exclusive use for social media sharing and personal web sites. No commercial use is permitted. Images may not be modified, cropped or the watermarks removed. No printing is allowed from these files.
- High-Res Digital Files: I understand these digital files are licensed for printing in sizes not to exceed 8” x 12”. Minor cropping is allowed to fit various print formats. These files may be used on commercial websites for the individual licensee. Copyright is retained by the studio and this license is not transferable.
- I understand that Ever After, LLC accepts cash, check, MasterCard, Visa, Discover, and American Express.
- Should I elect to pay by credit card, I authorize Ever After, LLC to charge to the card any fees (including a 3% processing fee), products or services purchased.
- Should I elect to pay by check, I understand there is a $35 fee for any bounced check, and I agree to pay for my order and bounced check fee in cash.
- In support of Ever After, LLC’s ability to work on my behalf, I agree that I will not attempt to negate, avoid, evade or circumvent the no refund policy in any manner with respect to this order. I agree not to dispute the agreed upon charges or otherwise seek a chargeback from the company whose credit card I have used to make this purchase (as it pertains to the no change or cancellation policy). I agree to be responsible for the total amount of this order, and to allow Ever After, LLC to charge any outstanding balance due (including for returned checks) on any credit card number my family has supplied and/or by electronic debit to my banking account(s), and to allow those charges in partial or full payments over any period of time. I agree that the order creates a legally collectable and reportable financial obligation. I agree that should any payments need to go to collections, I will be liable for any collection costs associated with the collection of the balance due.
- In the event that my Primary Card is declined, and a secondary card is provided, I agree to have the payment charged to my Secondary Card.
- I understand that once a payment has been processed, it reflects my agreement with the services rendered. (In the unlikely event of a payment dispute initiated with your financial institution, Ever After is committed to resolving the matter amicably. However, should a resolution not be reached, Ever After reserves the right to pursue appropriate legal channels to recover any incurred costs. We are always here to discuss any questions or concerns you may have.)
- I understand that any ordered portraits will be archived and be stored in a digital library for emergency replacements, and available for the next year. Anything not ordered today will be deleted at the studio's discretion. Portrait product order delivery times vary but that products ordered may be delivered within approximately 4 to 8 weeks after final payment is received. (Please allow for shipping delays beyond our control.)
- I understand that Ever After's liability for any data that is lost, damaged, stolen or destroyed is limited to the refund of payments received and a complementary reshoot of the portrait session.
- I agree that my initials and signature indicates my agreement to all of the above, and that I have reviewed my order and I agree that it is correct, that it is a final and custom order, and must be paid in full before any part of my order will be released by the studio.

DON'T FORGET: UNCHECK THE "INSTALMENT CALCULATION" BUTTON BEFORE SUBMITTING THIS FORM!!!!

FIRST, ensure that the CORRECT AMOUNTS are in these fields:
1. Total Order Amount
2. Payment Amount (today's payment)
3. Down payment amount paid at time of order
Then "Balance Remaining" shows the amount that is being put on layaway.

After the Down Payment, if the CLIENT wants to
MAKE A CERTAIN NUMBER OF PAYMENTS, LIKE 6
FOLLOW THESE STEPS:
First, ENTER 6 INTO THE "Total # Instalments" field to get an idea of what 6 fairly equal payments should be.
(We do this even though the 6th payment will be put in the "Last Instalment Odd Amount"!)
For our example, Balance Remaining is $3736.95.
Entering 6 in the "Total # Instalments" field gives us an "Instalment Amount" of $622.83.
So we know the last payment should be around $600.
Now, ENTER $600 in the "Last Instalment Odd Amount" field.
Then CHANGE THE "Total # Instalments" field to 5!
Look at the "Total Instalments + Last Instalment Odd Amount" field to
make sure it MATCHES the "Balance Remaining" field.
Adjust the "Last Instalment Odd Amount" if necessary to make sure "Total Instalments + Last Instalment Odd Amount" MATCHES the "Balance Remaining" field.

After the Down Payment, if the CLIENT wants to
PAY A CERTAIN AMOUNT MONTHLY, LIKE $500 PER MONTH,
rather than making a certain number of payments,
USE THIS FORMULA:
"Balance Remaining" field
divided by
the amount Client wants to pay (such as $500)
FOR EXAMPLE:
Balance Remaining of $3,736.95 / $500 = 7.4739.
$500 times 7 = $3500
$3500 minus $3736.95 = $236.95
So enter $236.95 into the "Last Instalment Odd Amount" field.
Enter 7 in the "Total # Instalments" field.
7 instalments of $500
plus 1 odd amount of $236.95
(for a total of 8 payments made)
= $3736.95
Look at the "Total Instalments + Last Instalment Odd Amount" field to
make sure it MATCHES the "Balance Remaining" field.
